The North Simcoe Muskoka Community Care Access Centre and Community Connection have formed an exceptional partnership to create a single, seamless solution to public information and referral services in the North Simcoe Muskoka region.
Information and referral services provide a connecting point for individuals to access services they need (community, social, health and related government programs), and are offered through several channels to ensure people receive the help they need. These channels include a 24/7 multilingual telephone help line, online searchable directories, as well as online chat and email options.
The partnership is building upon the existing capacity and professional information and referral infrastructure already established by Community Connection. This partnership eliminates the duplication of activities required for the delivery of public information and referral services, creating a cost effective information and referral system in North Simcoe Muskoka. The services provided through this partnership will be evaluated and monitored on an ongoing basis to ensure the Information and Referral system in North Simcoe Muskoka meets established standards and best practices.
The North Simcoe Muskoka Community Care Access Centre provides an access point to health and personal support services to help individuals live independently at home or to assist them in making the transition to a long-term care home. The North Simcoe Muskoka CCAC is one of 14 in Ontario working together in the community to enhance access and coordination for people across the province. Their Client Care Coordinators are regulated health professionals with specialized knowledge and skills that help to arrange needed services.
The North Simcoe Muskoka CCAC serves parts of Simcoe County, Muskoka, and Grey County, and works closely with their community health care partners - from the Local Health Integration Network, to physicians, hospitals, Family Health Teams, health care agencies and community support services - to enhance access to community-based health services for people across the province.
Community Connection has decades of service in the field of community information and referral and community database management. In 2005 Community Connection received their designation as the 211 service provider for Central East Ontario, a region of approximately 1.4 million residents. 211 (like 911), is a three digit phone number to access free, confidential, multilingual information about community, social, health and related government services available to residents. The 211 service will be available to all residents in Ontario by 2011.
